Keck Hospital of USC, is a 401-bed academic hospital located in Los Angeles, California. It is part of the University of Southern California’s medical enterprise, Keck Medicine of USC. We are a tertiary care hospital, without an emergency room and patient volume primarily coming from elective and emergency transfer surgical cases. Our patients are unique and we treat some of the most complex cases in the nation with a hospital average CMI of 3.1. In 2014, our Department of Surgery and key physician leadership developed and implemented a Blood Utilization Operating Room Dashboard. Despite this effort and implementing lessons learned with service lines, we realized we continued to rank below our target for optimal blood management. Understanding that we needed a more dedicated focus on PBM, the Medical Staff made a commitment to enhance our program and change the culture around blood utilization. As a result, in September 2018, eleven members of our faculty and staff, including representatives from hospitalist medicine, surgery, anesthesia, critical care and administration, attended the SABM Annual Conference and the PBM Certificate Course. Keck also became a Premier Hospital Affiliate during this time.

The SABM Conference and subsequent SABM Hospital Affiliation marked the kick-off of a robust PBM Improvement Collaborative, uniquely endorsed by the Medical Executive Committee and hospital administration, with these 11 members serving on the Steering Committee. Our goal with the PBM Improvement Collaborative is to provide a data-driven process improvement platform, led directly by primary stakeholders to transform the transfusion culture for both surgical and non-surgical patients alike. This PBM Improvement Collaborative has five working groups: Medical Management, Pre-Op Strategy, Intra/Post-Op Strategy, Education, and Data Collection & Reporting. These five working groups have leaders from different specialties, and the PBM cause has garnered wide support from all throughout the health system.

Inspired by many lectures at the SABM conference, we began to craft Phase 1 data metrics around which our improvement efforts are based. Since October 2018, and for the first half of 2019, we are focused on developing a data dashboard and have recently entered beta-testing for our PBM Dashboard – a feat we have been attempting to accomplish for several years as an organization! We are able to see blood utilization data now by physician and service line, and can give a hemoglobin trigger range for each of our providers. Our SABM Hospital Affiliation allows us to connect to educational, research and mentorship resources.

SABM has been the catalyst for our PBM Improvement Collaborative, and our focus in 2019 is to use our new-found data to change the culture around blood utilization. This furthers the goal at Keck Hospital of USC and to bring safer, higher quality care for our patients.
